Named in honor of the star ingredient, formaggio, Forma has set the stage for simple, Italian dining on the Westside. Set in a sleek yet inviting space, Forma features a cheese bar, extensive cheese selection and modern Italian menu. The culinary centerpiece of service is the Dalla Forma preparation in which freshly cooked pasta is finished in large cheese wheels of Parmigiano, Pecorino, Romano or Pecorino Gran Cru.

Incredible! Loved the huge cheese selection and the Dalla Forma was amazing. Awesome wine list too. Cannot recommend enough!!

New and trendy butVERY loud and overpriced....food excellent but small portions and average entree $30. Everything a la carte...even a veggie. Won’t go back.

Great menu, wine and cheese selection. The venue is small, not intimate but very pleasant. The food is very nicely prepared. It is not your typical Italian establishment. They put a spin on it treat us both unique and tasty. I would recommend it to anyone who is in the area. I felt the price and portions were not connected. It was worth the money but I think a bit more on the plate would be better.

Strange menu ....very expensive. Service was excellent. Would not return. Great if you like cheese ... but ... need more healthy items ... and charging you for bread at the table? Really? Really?

Everything was delicious- beautiful location and restaurant. The cheese fettuccini was amazing as was the salad we chose. Arancini appetizer was also excellent. By far the best place we ate during this trip.

Great for date night, ambiance is fantastic. Food is delicious. A bit overpriced, can have delicious Italian food for cheaper but ambiance won't be as great.

I’ve already recommended Forma to family members and friends! The food was so tasty and yummy. I had the TORTELLINI tossed in bella lodi cheese. And my fiancé had the FETTUCCINE BOLOGNESE tossed in parmigiano reggiano cheese. The service was outstanding. Can’t wait to go back.

Forma's Cacio e Pepe was the best I've ever had. Almost went back the next day for more!!! Looking forward to the another Forma experience soon.
